Know of someone with COVID-19 who might be interested in participating? Please fill out the form and we will contact them as soon as possible.
Should the person referred be eligible and accept to participate, we shall make a 25$ donation to one of the following community organizations*:
Support for seniors everywhere in Quebec
Street work and community support in Quebec
Protecting Natural Environments in Appalachian Corridor
* Note that there is no affiliation between the PROVID-LD research project and the organizations chosen for donations.

Person referred may partake if they meet the following requirements:
Not sure that all these requiremenst are met? No worries, we shall validate their admissibility upon reaching out to them.
The person resides in Quebec and is 18 years of age or older;
She will be in Quebec for the next 60 days;
She has tested positive for Covid-19 within the last 10 days (PCR test or home test);
She experiences at least one symptom of the disease according to the Quebec self-care guide;
She takes care of herself at home;
She is able to take medication on her own;
She has access to a phone and internet at home;
She is not pregnant or does not plan to become pregnant in the next few months;
She is not currently breastfeeding.
